Initial Evaluation

Service Description

This is your first appointment face-to-face with me. It is a 90 minute evaluation done in the comfort of your own home. I bring all of the tools and equipment with me that I will need. In this session, we will talk about your medical history, symptoms, complete an external and internal exam, develop goals for physical therapy, create a home exercise program, and estimate your plan of care.
